The Effectiveness of Stapled Hemorrhoidectomy

Abstract
The circular stapled hemorrhoidectomy is a newly introduced treatment modality for hemorrhoids. The stapled hemorrhoidectomy has been known to minimize the postoperative pain. However, there are still controversies on the indication and safety of the procedure and there are only limited numbers of study on the stapled hemorrhoidectomy in Korea. Therefore, this study was aimed to prove the clinical efficacy of a stapled hemorrhoidectomy. This prospective study analyzed 75 patients who underwent a hemorrhoidectomy from January 2006 to June 2008 at Dongguk University Ilsan Hospital. Among the 75 patients, 35 patients underwent a stapled hemorrhoidectomy and the Ferguson¡¯s closed hemorrhoidectomy was done for the remaining patients. The outcomes of the procedures including pain intensity scoring, amounts of analgesics required, time off work, Procedure related morbidity and satisfaction of patients were evaluated. Stapled hemorrhoidectomy was supehor to Ferguson¡¯s hemorrhoidectomy in the aspects of operative time, postoperative pain intensity and the amount of analgesics required and return to work (p < 0.05). At the patient interview which was done 3 months after operation, the satisfaction of patients tended to higher in stapled hemorrhoidectomy group although it was not statistically significant. Neither groups showed significant postoperative complications except one postoperative bleeding case with stapled hemorrhoidectomy. The stapled hemorrhoidectomy generated less postoperative pain, less time off work. Also, the complications related to this procedure were not much higher than those with a conventional hemorrhoidectomy. Therefore, the stapled hemorrhoidectomy may be a good surgical modality for hemorrhoid considering the lots of advantages.
